Please print or write out the appropriate form , fill it out and send with your ecu

If there is not a question on the form it is because it does not apply. Do not fill out a different form

This form not only gives me the information I need to flash your ecu properly, it also identifies your ecu to me and if your package is lost

This is how you communicate your flashing needs to me. If it is with your ecu , I DO NOT WANT OR NEED IT EMAILED TO ME.
